In order for you / your child to be included in the JLDA Live show at the Pavilion Theatre, Bournemouth, on Sunday 13th March 2022, please complete the form below, no later than Monday 15th November 2021.

Failure to complete the form in full and submit it, will mean you / your child will not be allowed to perform.
